    Vaccines Available: Chicken Pox, Dtap, Hepatitis A, Hepatitis B, HIB, HPV, Meningococcal, MMR, Polio, Pneumonia, and Tdap. 

    A printed current vaccine record is required, and a parent or guardian must be present.

    The following forms of payment will be accepted:

    Medicaid/Chip - No charge for vaccines. You must provide a paper photocopy of the card.

    Aetna, Blue Cross Blue Shield, Cigna, Humana and United Health Care PPS plans - A paper photocopy of the insurance card and your driver's license must be provided to receive vaccines at no charge. 

    Uninsured - $10 per vaccine ($40 maximum per child) Cash or Check only
